Remission of diabetes through lifestyle modification

By Dr Attia Anwar

In a few recent studies in newly diagnosed type 2 diabetic patients, it is shown that remission is possible through intensive lifestyle modification and if weight gain is avoided remission can be sustained.
The main interventions for this purpose are a whole plant-based diet and moderate exercise. There are five components of lifestyle medicine increased physical activity, healthy eating consisting mainly of a plant-based diet, adequate sleep, stress management, and avoidance of harmful substances like alcohol and tobacco. If we carefully examine all five components of lifestyle medicine, we will see that they play a necessary role in the prevention and management of diabetes and other non-communicable diseases. Lifestyle medicine is relatively new to many physicians and patients but it is the fastest growing field of medicine. It does not replace conventional medicine rather it makes it work better by focusing more on scientifically and medically proven non-pharmacological approaches towards the treatment of disease. There are proven strategies that can produce remission of diabetes without drugs.
Lifestyle modification not only plays a role in the management of blood sugar but also prevents the complications of diabetes such as diabetic retinopathy, neuropathy, and foot ulcers. This is very important for overall well-being, especially in developing countries where amputees have to face stigma, poor general well-being, and loss of economic power.

Type 2 diabetes can be reversed by healthy living on a whole plant-based diet, weight control, having good sleep hygiene, and avoiding stress as much as possible. These strategies should be added to the optimal treatment of diabetes. Every newly diagnosed diabetic should be given a trial of healthy living before starting pharmacological management. It is also important for the pre-diabetic or people with borderline glucose levels so that they should not progress to full-fledged diabetes.

Modification of lifestyle is the first line of treatment for diabetes mellitus. Weight loss is necessary for type 2 diabetes mellitus and hypertension related to obesity. Exercise and healthy eating are needed to lose weight and both of them are pillars of lifestyle medicine. When we do physical activity and exercise there is physical stress on the body which results in the transportation of glucose to the increased demand of the body. It causes improvement in blood sugar by increased intake of glucose, increased utilization of glucose, improved insulin sensitivity, and protection of the function of insulin-producing cells of the pancreas. Exercise recommended for all adults is at least 150 minutes of moderate physical activity per week.
Remission of diabetes means that the patient does not need medications for a certain period till their blood glucose stays normal. Maybe he never needs them if he sticks to a healthy lifestyle. This is very important and proved by research studies. Although it has long been proven that lifestyle modification is the first line of treatment for diabetes and pre-diabetes. However, most clinicians fail to stick to lifestyle modification. Patients especially in our region expect doctors that he should write a prescription for medicine, simple advice is not enough for them. They think that the doctor’s job is to give them medicine rest of the things they can do on their own. That is the reason, even sometimes doctor who is totally convinced that this patient does not need medications writes some harmless multivitamins that are also not required.

This remission of diabetes is going to give us hope. If we can reverse a major disease by bringing some changes in life, patients will be off medicine. This will be a breakthrough in convincing the whole population. Every medicine has its side effects. However, the lifestyle changes that are recommended for the reversal of diabetes are very useful changes. They will improve your general health and will also prevent many other diseases. The main stress is on diet and physical activity to lose some weight and maintain a healthy weight. But if we combine the other three pillars of lifestyle medicine like adequate sleep, avoidance of stress, and abstaining from harmful substances like alcohol and tobacco. It is like the cherry on the cake that it is going to improve your blood glucose further and reduce the risk of heart attacks and stroke.
It is very important that we counsel our patients and tell them about all options of treatment along with side effects, that are available, and help them to make informed choices. That means we should use the approach “skills before pills” for everything. If it is possible to avoid pills we should avoid them and help our patients to adapt to a change in living by scientific methods. In this way, they can benefit from recent research, and the burden of diseases and medicines is reduced. Then we should monitor and help them to maintain that change. This will be beneficial for individuals and then overall for society.

Mohsin Naqvi slams terrorist attacks in Musakhel and Kalat

ISLAMABAD, Pakistan: The Federal Minister for Interior Mohsin Naqvi on Monday strongly condemned the incidents of terrorism in Balochistan’s Musakhel and Kalat areas.

Earlier in the morning, terrorists armed men offloaded 23 passengers from trucks and buses in Rarasham area of Musakhel district in Balochistan, and opened fire on them leaving them dead.

In a separate incident, armed men attacked Levies and police stations in various area of Balochistan including Kalat last night which resulted in killing of five people.

In his statements on Monday morning, the interior minister said that terrorists showed brutality by targeting innocent people near Musakhel. He said that terrorists and facilitators will not escape the fateful end.

The minister said that people from every walk of life along with the security forces have made immeasurable sacrifices in the war on terror.

Mohsin Naqvi also paid tribute to the martyred police and levies personnel, expressing regret over the martyrdom of five civilians in the Kalat attack.

The interior minister expressed heartfelt sympathy and condolences to the families of the martyred officials and citizens in both the terrorist incidents. The minister prayed for the speedy recovery of the injured.

The minister said that the sacrifices of martyrs will always be remembered. He maintained that the enemy is making a heinous conspiracy to destabilize the Country.

“With the strength of unity, evil intentions will be thwarted,” he said.

Will Mahrang Baloch, Western diplomats and Human rights defenders condemn Musahkhel incident?

Islamabad, Pakistan: Once again, terrorists killed bus passengers in Balochistan after checking their National Identification Cards (IDC) and executed 23 who belonged to Punjab. This is not the first incident of such nature rather it is a routine of TTP and BLA (Pakistan state now calls them Fitna al Khawarij) to attack buses, shops, and working places; check the IDC of persons and kill those who belong to Punjab or a particular sect. TTP does it on sectarian background and kills people belonging to a particular sect while BLA does it on ethnic background meaning both work closely to divide Pakistan on ethnic and sectarian backgrounds.

Over 7,000 non-Baloch teachers (between 2006-2022) were reported to have sought transfer to educational institutions in Pashtun-majority areas of Balochistan or resigned and migrated to other provinces and Baloch separatists claimed responsibility for murdering academic staff and tried to justify their acts as revenge against the state of Pakistan.

Unfortunately, a political cadre of the country that provides moral support to separatists like BLA does not condemn BLA when Punjabis or Sindhis are killed on ethnic background nor does the diplomatic cadre living in Islamabad comment as both overtly and covertly support separatists and provides them support and visas to leave Pakistan for western world from where they operate against Pakistan.

Will Mahrang Baloch, Western diplomats and Human rights defenders condemn Mushkhel incident?
Will Mahrang Baloch, Western diplomats, and Human rights defenders condemn the Mushkhel incident?

A journalist, a doctor, an engineer, a businessman, an academician, a scholar, a government official, or a writer cannot get the visa as easily as a terrorist of BLA gets a visa from EU and North American countries. This is a hard fact that people like the late Karima Baloch or late Sajid Hussain got visas even not appear for their interviews and thousands like such enjoy this visa service if they can establish that they are separatists and want to create a free Balochistan. If you want to visit North America or European countries, you need to try to establish your profile as ‘anti-Pakistan separatists’ and you will surely get the visa later or sooner. First, you will be provided a visa and then you will be eligible for asylum when you reach the country of your desire.

After the tragic incident of Musahkhel where BLA terrorists killed 23 passengers after offloading from the bus, will political and human rights mouthpieces such as Mahrang Baloch, Mahmood Khan Achakzai, and many such them condemn BLA that have already taken responsibility for this attack? Will Human Rights defenders condemn this action of BLA?

The same-day attack in Kalat and other places in Balochistan determines that terrorists are collaborating again and have something bigger in their minds now.

The Faiz Factor: PTI is crumbling while Aleema Khan tries to manage

Peshawar, Pakistan: The last 30 days had been ‘terrible’ for former prime minister Imran Khan and his family after ‘the Faiz Factor’ ceased to work, consequently Khan’s contact with the world from jail is not as free as it had been.

The man who used to instruct the PTI for political moves is under military custody as he is facing a Court Martial and the rest of the PTI leaders face a trust deficit with Imran Khan. Faiz Hameed was the only guide of Khan who was trustworthy for him and whose instructions Khan followed cent percent.

The leaked alleged audio call between former Prime Minister Imran Khan’s sister Aleema Khan and PTI’s central spokesperson Rauf Hasan explained what kind of crisis the party had faced due to family differences after the party’s internal differences. It has been said that PTI is confronting a severe organizational crisis, intra-party grouping but top of it is the alleged role and interference of former First Lady Bushra Bibi. PTI circles claim that intra-family disputes threaten the party more than intra-party disputes and internal mistrust of this family. It is because the effects of the internal disintegration that the party is going through have left the workers frustrated. These circles believe that the ongoing tussle between the sisters of Imran Khan and his wife at a critical time is a disastrous element and the party has become a “sandwich” between them. Circles claim that in the past Faiz Hameed had been managing these family disputes and Bushra Bibi used to get guidance from him and now she has no one to guide her. It is pertinent to mention that contact with Bushra Bibi and Faiz Hameed continued even after she was jailed through a networking Faiz Hameed had been running within Rawalpindi Jail. It is alleged that Faiz Hameed and Bushra Bibi were managing the social media team of PTI sitting in the United States and the arrest of Faiz Hameed has also created a gap between the team and Imran Khan.

Regarding the KP government of PTI, corruption charges, abuse of power and nepotism are some of the allegations that the Chief Minister of Khyber Pakhtunkhwa is facing from workers of the party and he is under severe criticism for his dubious and controversial character and Aleema Khan has also targeted him after he postponed Islamabad public meeting. She accused CM Gandapur of working for some sections of the establishment and Aleema believes Chief Minister Ali Amin Gandapur has extraordinary “patronage” of Gandapur and Bushra Bibi.

Aleema khan and Faiz Hameed

In the background of all this situation, it can be said that on one hand, this party is suffering from serious problems of internal differences and family mistrust.

Party circles claim that these are reasons that pushed Aleema Khan to meet disgruntled workers in Peshawar and Mardan without informing Chief Minister Ali Amin Gandapur and cabinet members. The workers piled complaints against the Chief Minister and other top leadership in their meeting with Aleema Khan. In the meeting with workers of constituency PK-83, MNA Shandana Gulzar and PTI leader Khadija Shah were also present along with Aleema Khan. According to sources, Aleema Khan expressed her displeasure with the way the party is run in KP province. After meetings in Peshawar, Aleema Khan also visited Peshawar tehsil Gorghathari and Mardan to listen to the grievances of workers. It looks like the responsibility of the party has to be shared by Aleema Khan now after the arrest of Faiz Hameed who was the mentor of the party after Imran Khan was arrested.

‘UN Report proof of Israeli genocide against Palestinians’, says PM Shahbaz Sharif

ISLAMABAD, Pakistan: Prime Minister Shahbaz Sharif has demanded immediate action from international community on the recent report of the UN Office for the Coordination of Humanitarian Affairs (OCHA) regarding the forced eviction of Palestinians from their land and child malnutrition.

In a statement, the prime minister said that the latest UN report is fresh evidence of the Israeli genocide against the oppressed Palestinians.

The prime minister said that the UN report testifies that there is not a single day in which innocent Palestinian blood is not shed. It says that the Palestinian children are the biggest target of Israel.

The prime minister said that the forced eviction of 250,000 Palestinians from their homes in the month of August is a challenge to international institutions, international conscience and international law. He said that the pace of Israeli oppression can be gauged from the unjust eviction of 146 Palestinians from their homes in just two days.

Shahbaz Sharif said that the report of the United Nations agency confirms that on the one hand, the genocide of Palestinians with deadly weapons is continuing while on the other hand, hunger is being used as a weapon. Only one percent of children in northern Gaza and six percent in southern Gaza are receiving food, he added.

Expressing serious concerns over the massacre of Palestinian people, the prime minister said that reports of international organizations are a serious indictment against Israel. He said the murderers of humanity should be punished and the oppressed be protected.

The prime minister expressed Pakistan’s commitment to speed up delivery of food to Palestinians, especially children.
